Summary:

Lack of insight has been accepted as one of the most important features of schizophrenia. Between 50 and 80% of the patients have been shown to be partially or totally lacking insight into the presence of their mental disorder. Previously, insight in psychosis had been regarded as a categorical concept. However, more recent authors define insight as a multidimensional concept. Lack of insight leads to several consequences: increased rate of relapse, increased rate of re-hospitalization, heightened risk of violence, greater symptomatic impairment, poorer functioning and vocational attainment and a failure to recognize the need for treatment, leading to treatment non-adherence (probably one of the most important consequences). In addition, higher levels of insight in schizophrenia have been associated with depression and hopelessness. The current article describes the concept of insight and its impact on symptoms and functioning. 

Conceptul de insight în schizofrenie

Lipsa simptomatologiei este consideratã una dintre cele mai importante trãsãturi ale schizofreniei.
Între 50 si 80 % dintre pacienti nu îsi constientizeazã sau constientizeazã partial prezenta propriei afectiuni psihice. Dacã initial insight-ul a fost considerat un concept în psihoze, în ultimul timp el este vãzut ca un concept multidimensional. Lipsa insight-ului are mai multe consecinte: cresterea ratei de recãdere a bolii, cresterea numãrului de spitalizãri, cresterea riscului de violentã, agravarea simptomatologiei, scãderea capacitãtii de functionare si diminuarea capacitãtii de recunoastere a necesitãtii tratamentului, ceea ce duce la non-compliantã (probabil una dintre cele mai importante consecinte). În plus, nivelurile crescute de insight sunt
asociate cu depresie si lipsa de sperantã. Articolul fatã descrie conceptul de insight si impactul lipsei constientizãrii bolii asupra simptomatologiei sI evolutia pacientilor cu schizofrenie.
